Primary Care Services in English & Haitian Creole

Primary Care & Preventive Wellness Visits

A primary care provider who knows your history makes every visit count. Daniella provides ongoing care for adults and older adults, from annual physicals and Medicare Wellness Visits to long-term management of high blood pressure, type 2 diabetes, high cholesterol, and other chronic conditions. You’ll review screenings, medications, and lab results together, and leave with a clear, personalized plan for staying healthy.

Weight Management

Lasting weight loss is about more than a number on the scale. We take a medical approach that looks at your overall health, metabolism, medications, and lifestyle, then build a realistic plan with you. Care may include nutrition guidance, activity goals, regular check-ins, and prescription weight-loss medications when appropriate. The goal is steady progress you can maintain, and better health along the way.

Same-Day Sick Visits

When you’re sick, you shouldn’t have to wait weeks or spend hours in urgent care. We see patients for common illnesses, including colds, flu, sinus and ear infections, sore throats, urinary tract infections, and stomach bugs, often on the same day or next-day. Telehealth visits are also available for many concerns. If your symptoms need emergency care, we’ll tell you right away.

Menopause & Women’s Health Care

Perimenopause and menopause can bring hot flashes, poor sleep, mood changes, and more, and you don’t have to just push through them. We offer personalized care for menopause symptoms, contraception counseling, preventive screenings, and everyday women’s health concerns. You’ll get clear answers, real options, and a provider who takes your symptoms seriously.
